The oldest surgery performed for urinary stone was perineal lithotomy (open surgery for stone in urethra) in 12th century and pyelolithotomy (open surgery for stone in kidney) in 1879. The trend continued for many centuries. But 1976 was the year which saw the advent of PCNL (Percutaneous Nephrolithotomy). This operation reduced the need for open surgery, which is now reserved for exceptional indications. PCNL is now a choice of treatment for large or complicated stones or stones that cannot be extracted with other procedures.

Within the span of eight years, in 1984, advancement was made, and Extracorporeal Shock Wave Lithotripsy (ESWL) was introduced. This was a major innovation, perhaps one of the most significant medical advancements of the century. ESWL is currently used in the treatment of uncomplicated small stones located in the kidney and upper ureter, with less stone mass. Ureteroscopic stone removal (URS) was the next achievement for the treatment of kidney stones. It is a very safe procedure in experienced hands. No incision is required for this procedure. The only limitation of this procedure till some time back was the rigidity of the scope which could reach only up to the upper part of ureter. But this limitation was eliminated when flexible ureteroscopes were invented, which could easily access stones anywhere in the entire urinary tract. Flexible URS is also termed Retrograde Intra Renal Surgery (RIRS). It is the most popular modality of treatment of urinary stones.
